With reference to the said notice we would like to state that we have uploaded Form 5 for
F.Y. 2021-22. We had applied for ‘Application For Change’ on 14/10/2021 for which we had
done all the necessary compliances for obtaining revised certificate. While doing the same we
had uploaded Form 1, Form 2, Form 3 dated 14/10/2021, the limit of which is still not
exhausted. So there is no need to revise Form 1, Form 2 and Form 3.

We could not upload Form 5 for F.Y. 2021-22 within due date as there are so many statutory
compliances to take care of such as Income Tax, GST, Municipal Corporation, etc. We were
about to upload Form 5 but there was delay for the same.

Please take the above details on your records and oblige.
