           2)     I am applying for change/ transfer of the separate designated bank account
                  bearing No.341601010035030 at Navi Peth Branch having IFSC No. U8IN0534161
                  In Union Bank of India bank to Kabra Sankul Old Bus Stand Navi Peth branch
                  having IFSC No. MAH80000004 in Bank of Maharashtra bank bearing Account No.
                  50462251875.

           3)     I Say, declare and undertake to transfer Rs. 26400/- (Rupees. Twenty Six Thousand
                  four hundred) being the closing balance in the previous designated bank account
                  bearing No. 341601010035030 to the new dqsignated bank account bearing No.
                   60462257875. Within 15 days post approval of the Authority for change/ transfer of
                  the existing designated bank account to the new designated bank account.
